This is the link to the Dagorhir Waiver. You need one filled out no matter what for Maethodoron. But if you are under 18 years of age, you have to have one filled out, signed by a parent and notarized.
As far as i know, its about $5 to have one notarized. So you need to do this before going to any large events.
